Incare’s care plans are designed and managed by Registered Nurses. A nurse will periodically also check on your loved one to make sure they’re getting the care that’s best for them.

Who it’s for
Nursing visits are important to ensure that the care provided is comprehensive and addresses the unique needs of the patient. Every Incare patient gets an initial nursing visit and follow-ups every six months. Further supplementary visits can be arranged for those who need more help managing their health.
What it looks like
At the initial assessment, a nurse will:
- Review medical history
- Review medications
- Conduct a physical assessment
- Develop a personalized care plan
At quarterly visits, a nurse will:
- Conduct a physical assessment
- Address recent changes or issues
- Review medications
- Discuss any issues with your loved one’s caregiver
- Update a care plan for the next six months
At supplementary visits, a nurse can:
- Conduct a wellness check
- Assess diet and nutrition
- Monitor and manage your loved one’s medical condition
- Manage medications
- Administer intravenous and injectable medicines
How it works
Your loved one will have an in-home assessment from a nurse at the start of their care. This helps us learn your loved one’s unique needs so we can design a plan that’s right for them. A nurse makes follow-up, quarterly visits to review and update your loved one’s care plan. We also arrange supplementary visits, when needed.
