When it comes to home health care in NYC, finding a provider you can trust is essential. Whether your loved one is aging, managing a disability, or recovering from an illness, the right home care makes all the difference. At Incare, we offer consistent, quality-oriented services designed to meet your loved one’s needs—right in the comfort of their own home.
Personalized Home Care with a Human Touch
Choosing home health care is a personal decision. At Incare, we believe every client deserves care that respects their lifestyle, preferences, and values. That’s why our team works closely with families across New York City to create tailored home care services that promote independence, dignity, and safety at every stage of life.
We provide flexible support—ranging from a few hours a week to 24/7 care—throughout four boroughs of NY. Our goal is to help your loved one live the life they envision with compassionate, hands-on help.
Home Health Aides Who Care Like Family
Incare’s trained home health aides offer more than just assistance—they bring peace of mind. Whether your loved one needs help with grooming, dressing, bathing, or preparing meals, our caregivers ensure comfort while supporting daily routines. They also assist with light housekeeping, medication reminders, mobility, and keeping appointments.
These personalized services make daily life easier and safer—especially for those living with long term care needs, chronic illness, or age-related challenges.
Companion Care for Emotional Wellbeing
Living alone can impact emotional health. Our home care includes warm, engaging companion care that helps ease loneliness. Companions spend quality time with clients, whether that means going on walks, playing games, attending social events, or simply chatting.
This kind of emotional support can greatly improve the quality of life for those facing disability, isolation, or cognitive decline.
Skilled Nursing Care Through Trusted Partners
Incare works alongside registered nurses and licensed practical nurses to enhance our home health care offerings. Through scheduled nursing visits, these professionals oversee the care plan, conduct physical assessments, and help manage medical conditions.
These services ensure that health needs are addressed promptly and accurately in the patient’s own home.
Safe and Supported Transitions from Hospital to Home
Transitioning home after a hospital stay can be overwhelming. Incare’s transitional care services help families navigate this shift with ease. Our caregivers assist with post-discharge instructions, medication reminders, follow-up appointments, and daily tasks—reducing the risk of readmission and promoting recovery.
This is especially valuable for patients returning from rehab or dealing with new diagnoses and complex care routines.
Specialized and Long Term Home Care Services
For clients with conditions such as Parkinson’s, heart disease, arthritis, cancer, or stroke recovery, Incare offers personalized care tailored to their specific needs. These home health services allow individuals to remain safely and comfortably in their homes while managing chronic or complex medical issues.
This approach promotes independence and supports a higher quality of life, even with long-term or progressive health concerns.
Respite and Short-Term Relief for Caregivers
Family caregivers need breaks too. Incare’s respite care gives families the time to rest, travel, or simply recharge, knowing their loved one is in good hands. Whether it’s just a few hours a week or full-time coverage, we offer flexible relief so family caregivers can care for themselves as well.
This ensures continuity of care without compromising the wellbeing of your loved one—or your family.
Compassionate Memory and Hospice Support
For families managing dementia or Alzheimer’s, our memory care services are designed to keep clients safe, comfortable, and engaged. We also coordinate hospice care in partnership with experienced professionals to support clients with life-limiting illnesses and their families through compassionate end-of-life care.
Each plan is personalized to honor the patient’s dignity, preferences, and emotional needs.
Care Management and Community Support
Incare offers comprehensive care advisory and management to help families coordinate care, understand Medicare and Medicaid options, and access local resources. Our team liaises with elder advocates, nurses, and social workers to ensure seamless care—whether at home, in rehab, or during transitions.
We also help families determine what services their loved ones may be eligible for, simplifying complex healthcare systems and ensuring that every client’s unique needs are met.

Incare provides routine care through:
- Private Pay – direct payment for flexible services.
- Private Long-Term Care Insurance – with support for claim submissions and billing.

Is home care covered by Medicare or Medicaid?
Medicare generally provides only short-term, skilled nursing following hospitalization. Medicaid can cover ongoing personal care and aide services through MLTC and NHTD programs.
What insurance plans do you accept?
Incare works with a wide range of Medicaid Managed Long-Term Care (MLTC) plans as well as most private long-term care insurance providers. We can confirm participation with your specific plan when you contact our intake team.
Do you accept private pay and long-term care insurance?
Yes. We provide services through Private Pay, Private Long-Term Care Insurance, and Medicaid programs. Families without Medicaid coverage may still arrange services through private pay. Private pay rates in NYC typically range from $35–$38 per hour, depending on the type and frequency of care. We can provide a detailed quote after learning more about your loved one’s specific needs.
Do you help families navigate Medicaid eligibility?
Yes. While we do not determine eligibility, our team helps families understand the process, connect with legal and financial advisors, and coordinate services once eligibility is established. We also provide guidance during the Medicaid application process and connect families with resources to make the process easier.
How do you help families stretch their care dollar?
We’re experts in care planning. Our team helps families combine Medicaid, long-term care insurance, private pay, and concierge services to create sustainable, cost-effective care plans.

Yes. Every client receives a personalized care plan tailored to their needs, health conditions, and family preferences. Care plans are reviewed regularly and adjusted as needs change.
How flexible are your services?
Very. Families can schedule a few hours a week, daily shifts, or live-in coverage. Schedules and caregivers can always be modified.
Do you provide senior care for long term needs?
Yes. Many of our clients are older adults who benefit from long term services that promote safety, dignity, and well-being. Whether through personal care, companionship, or specialized support for memory loss and physical limitations, our caregivers provide consistent, high quality care that adapts to your loved one’s changing life and health needs, even as new challenges arise.

What is the application process for caregivers?
Caregivers can apply by submitting an application online or in person at our Brooklyn office. Applicants must provide valid identification, proof of work authorization, and required certifications. Once the application is complete, our HR team reviews the documents, verifies credentials, and schedules orientation.
What if a caregiver is unsure about which insurances Incare works with when transferring vendors?
If a caregiver is uncertain about the insurance plans we accept during a vendor transfer, they should contact our HR or Intake department. We will confirm accepted insurances on a case-by-case basis.
