The Incare Support to Home Concierge Program is a structured, care-based program designed to bring patients home safely and ease the stress of discharge day. We coordinate closely with hospital teams and families to support a smooth, comfortable transition home.

The program is designed for patients returning home from the hospital who need urgent or same-day in-home support, families seeking peace of mind, and hospitals needing reliable discharge assistance. It also helps anyone requiring support with personal care, medication reminders, meals, or mobility during recovery.
What it looks like
Flat-Rate Discharge Support — $249
Includes up to 6 hours with a trained, certified caregiver.
Caregiver Tasks:
- Safe escort home
- Settling-in support
- Light meal prep & hydration encouragement
- Safety-focused oversight during the transition
- Guidance for safe movement around the home
- Updates to families & hospital teams
Additional Options:
If continued care is needed beyond the initial six hours, Incare can seamlessly coordinate a full range of home care services. Our team will develop a personalized care plan, which includes a brief at-home nurse assessment, to ensure safe and appropriate ongoing support.
How it works
Once a hospital or family requests support, a certified caregiver is assigned based on the patient’s needs. The caregiver supports a safe transition home, assisting with personal care, medication reminders, meals, and mobility while providing comfort and supervision. Families and care teams receive regular updates throughout the visit, and additional support can be arranged as needed.
